A rotator cuff tear is a common shoulder injury affecting the tendons that stabilise the shoulder and allow you to lift and rotate your arm. When a tendon tears, it can cause pain, weakness, and difficulty with everyday activities such as reaching overhead or lifting objects.

Many rotator cuff tears can be treated without surgery using physiotherapy, activity modification, or injections. However, if symptoms persist or shoulder function is significantly affected, surgical repair may be recommended.
Rotator cuff repair surgery aims to relieve pain, restore strength, and improve shoulder movement so patients can return to normal daily activities.
Rotator cuff tears can occur suddenly after an injury or develop gradually as the tendon weakens over time. Risk factors include ageing, repetitive overhead activity, heavy lifting, and long-standing shoulder impingement.
While many tears can be managed with physiotherapy, activity modification, or injections, surgery may be recommended if pain persists, shoulder strength is significantly reduced, or the tear is large and affects daily activities.
During your consultation, your surgeon will assess your shoulder and may arrange imaging such as an MRI scan or ultrasound to confirm the diagnosis and determine the most appropriate treatment.
Rotator cuff repair is most commonly performed using arthroscopic (keyhole) surgery, a minimally invasive technique.
During the procedure:
Arthroscopic surgery allows the surgeon to repair the tendon with precision while minimising disruption to surrounding tissues.
This approach generally results in less postoperative pain, smaller scars, and faster recovery compared with traditional open surgery.
In some cases involving larger or more complex tears, a mini-open or open procedure may be required to complete the repair.
The aim of rotator cuff repair surgery is to relieve pain, restore shoulder strength, and improve movement so you can return to normal daily activities.
With appropriate rehabilitation, most patients experience significant pain relief, improved shoulder strength, and better range of movement, allowing them to return to normal daily activities, work, and sport.
First 2–6 weeks: Protecting the repair
Your arm will usually be supported in a sling to allow the tendon to heal. During this period, gentle physiotherapy exercises may begin to maintain mobility and reduce stiffness.
6–12 weeks: Regaining movement
As healing progresses, guided physiotherapy focuses on restoring shoulder range of motion and gradually increasing activity.
3–6 months: Strength and function
Strengthening exercises are introduced to rebuild shoulder muscles and improve stability. Many patients return to most daily activities during this stage.
6–12 months: Full recovery
The shoulder continues to strengthen and function improves further. Most patients achieve their final recovery during this period, particularly after larger tears.
More than 90% of patients achieve substantial improvement in pain and shoulder function following rotator cuff repair surgery.
