The best choice for hip replacement surgery

Our leading surgical team have perfected the Rapid Recovery modified Rottinger approach - a minimally invasive, muscle-sparing total hip replacement technique, designed as a day-case procedure that’s appropriate for a wide range of patients

Here’s why we think it’s the best choice:

This advanced surgical approach is designed to help you regain mobility quickly, often allowing you to walk within hours of surgery. By prioritising swift rehabilitation, it reduces downtime and speeds up your journey back to daily life.

Our modified Rottinger approach uses small incisions and innovative, streamlined techniques to minimise tissue damage. This means you experience less pain, reduced scarring, and significantly faster healing than traditional hip replacement methods.

Because we don’t cut through any muscles during surgery, this technique ensures improved stability, reduced discomfort, reduced risk of dislocation, and a quicker return to normal activities without compromising strength. Even with a standard “minimally invasive” approach, you can be out of action for many weeks. With our innovative modified Rottinger approach hip replacement surgery, many patients can be back to work and their usual daily activities within days.

With the Rapid Recovery approach, generally, you can leave the hospital the same day as your surgery, so you can enjoy a convenient and comfortable experience without prolonged hospital stays. Other hip surgeries can require up to 5 days in hospital.

Been told you’re not suitable for hip replacement surgery? As this technique requires less time under anaesthetic, this procedure can be suitable for individuals of various ages and conditions, including those with more complex medical conditions and those with high body mass index, ensuring more people can benefit from its advanced outcomes

Before our top international surgeons see you in the operating room, they will have run a computer simulation of your operation using the latest technology. This means they will have worked out any potential issues well in advance. During the operation both Prof Adrian Wilson and Dr Kristian Kley will be present, focusing entirely on you.

FeatureOur Rapid Recovery ApproachTraditional Posterior Approach
Incision & ApproachSmall 4–6 cm incision, placed midway between the front & side of the hip (anterolateral approach)10–20 cm incision, located towards the back of the hip (posterior approach)
Muscle PreservationAll muscles are preserved, leading to faster recovery & less painMuscles are detached & reattached, prolonging healing and increasing post-op pain
Surgical Time20–30 minutes90 minutes (average)
Time Under Anaesthetic20–30 minutes, reducing risks associated with prolonged anesthesia90 minutes
Hospital Stay98% of patients discharged within 24 hours2–5 days, depending on pain levels
Recovery TimeFaster recovery: Most patients walk unaided within hours, 50% can return to work and most activities within 2 weeks.Patients use crutches or frames for up to 6 weeks, must avoid lying on the operated side, and follow strict movement restrictions for 6 weeks
Movement RestrictionsNo restrictions post-surgery; patients can sit and lie as they like from day onePatients must avoid deep flexion (low sitting, lying on the operated side) for 6 weeks due to dislocation risk
Infection RateLower risk due to small incision, muscle preservation, and shorter surgical time; Kristian Kley’s rate: 3 infections in 7,000 cases (0.04%)UK national infection rate: 1–5%
Blood Clot RiskLess than 1% risk, including both deep vein thrombosis (DVT) and pulmonary embolism (PE)Higher risk according to the National Joint Registry
Dislocation RiskSignificantly lower due to preserved muscle & joint integrity; Dr Kristian Kley has never seen a dislocation in 15 yearsNational statistics: 2% of patients (2,000 per year) suffer dislocation, often requiring further surgery
Nerve or Vessel DamageMidway between major nerves (femoral & sciatic), reducing risk; Kristian Kley has never seen a nerve injury with this approachNational statistics: 0.5% risk of permanent nerve injury (500 patients per year). Lateral approach has a 15% risk of nerve damage leading to limping & disability
ScarringSmaller incision results in minimal scarringLonger incision leaves a larger, more tender scar

At your first consultation, Professor Adrian Wilson will be able to discuss your specific case and discuss the options with you.

The Rottinger approach is a well-established, minimally invasive technique for total hip replacements that has been successfully used worldwide. There have been numerous research studies published on this technique and has been shown to be safe and effective with some studies showing reduced complications and better function than other surgical hip replacement methods. Dr Kristian Kley has performed this procedure on over 3000 patients.

The Rapid Recovery approach has an excellent track record. Dr. Kley has been performing this procedure for over 15 years and performs 450 procedures per year. The vast majority of his patients are able to leave hospital within 24 hours with very little pain. Of the procedures Dr. Kley has performed 0% have had hip dislocations, there have been 2 cases of infection in the last 15 years with no deep infection since 2019. This is in contrast to the UK national statistics which quote a deep infection rate of between 1% - 5% and patients often needing to stay in hospital for 5 days to recover from a traditional hip replacement.

Modern hip replacement implants are designed to last a significant amount of time. The 10 year survivorship is close to 100% and there is now some excellent data showing implants lasting up to 20 years.

While the likelihood of major complications following a rapid recovery hip replacement is low, it’s important to be aware of the potential risks involved. Here’s what you need to know:

  • Infection: Though rare, infections can occur either at the wound site or around the implant. Under Dr. Kley’s care, the infection rate is impressively low, with only 2 cases in the past 15 years. By comparison, the national average for infections with traditional hip replacement surgery is 1-5%. Our rigorous protocols before, during, and after surgery help minimise this risk further.
  • Blood Clots: Any surgery carries a risk of blood clots, such as deep vein thrombosis (DVT) or pulmonary embolism. However, the Rottinger approach significantly reduces this risk by allowing patients to move sooner after surgery, as movement is key in preventing clots.
  • Dislocation: Nationally, the dislocation rate for first time  hip replacements is  2%. With the rapid recovery technique, there have been no recorded dislocations under Dr. Kley’s care.
  • Implant Loosening: Implants are designed to last at least 10 years, with many lasting up to 20 years based on recent data. A small percentage of all implants may loosen prematurely, but this risk remains very low.
  • Fracture: Although extremely rare, fractures can occur during or after surgery. If a fracture does happen, it is addressed during the procedure or with further surgical intervention if it occurs in the weeks following surgery. The chance of a fracture with this approach is significantly less than 1%.
  • Nerve and Vessel Damage: This approach reduces the risk of nerve or vessel damage by positioning the surgical field away from major nerves. Unlike anterior and posterior techniques, the rapid recovery approach avoids proximity to the femoral and sciatic nerves.

No recorded nerve or vessel injuries in the last 15 years atDr Kley’s practice having performed over 3000 hip replacements

With the rapid recovery technique, you will benefit from an advanced surgical procedure that prioritises safety and minimise risks, ensuring the best possible outcomes for you.

While recovery times can vary from person to person, many of our patients are walking unaided within a day. They can get back to work within a few weeks and get back to driving within a week or two.
