At Orthopaedic Innovations, our international surgical team has extensive experience in the Rottinger Approach technique for hip replacement surgery.
Originally, this surgical technique was developed in the 1930’s then modified into the Rottinger Approach in the 1960’s.
Over the past 15 years, Dr Kristian Kley and Prof Adrian Wilson have pioneered the further development of this technique (the Rapid Recovery hip procedure) to make it one of the safest and most effective surgical hip replacement procedures available globally.
Feature |
Our Mini-Invasive Anterolateral Approach |
Traditional Posterior Approach |
---|---|---|
Incision & Approach |
Small 4–5.5 cm incision, placed midway between the front & side of the hip (anterolateral approach) |
10–20 cm incision, located towards the back of the hip (posterior approach) |
Muscle Preservation |
All muscles are preserved, leading to faster recovery & less pain |
Muscles are detached & reattached, prolonging healing and increasing post-op pain |
Surgical Time |
20–30 minutes |
90 minutes (average) |
Time Under Anaesthetic |
20–30 minutes, reducing risks associated with prolonged anesthesia |
90 minutes |
Hospital Stay |
Majority discharged within 24 hours (one-night stay is standard) |
2–5 days, depending on pain levels |
Recovery Time |
Faster recovery: Most patients walk unaided within hours, 50% can drive within 2 weeks, return to work in 4–6 weeks |
Patients use crutches or frames for up to 6 weeks, must avoid lying on the operated side, and follow strict movement restrictions for 6 weeks |
Movement Restrictions |
No restrictions post-surgery; patients can sit and lie as they like from day one |
Patients must avoid deep flexion (low sitting, lying on the operated side) for 6 weeks due to dislocation risk |
Infection Rate |
Lower risk due to small incision, muscle preservation, and shorter surgical time; Kristian Kley’s rate: 3 infections in 7,000 cases (0.04%) |
UK national infection rate: 1–5% |
Blood Clot Risk |
Less than 1% risk, including both deep vein thrombosis (DVT) and pulmonary embolism (PE) |
Higher risk according to the National Joint Registry |
Dislocation Risk |
Significantly lower due to preserved muscle & joint integrity; Dr Kristian Kley has never seen a dislocation in 15 years |
National statistics: 2% of patients (2,000 per year) suffer dislocation, often requiring further surgery |
Nerve or Vessel Damage |
Midway between major nerves (femoral & sciatic), reducing risk; Kristian Kley has never seen a nerve injury with this approach |
National statistics: 0.5% risk of permanent nerve injury (500 patients per year). Lateral approach has a 15% risk of nerve damage leading to limping & disability |
Scarring |
Smaller incision results in minimal scarring |
Longer incision leaves a larger, more tender scar |
